Photograph Release Form

The Windsor Historical Society sometimes photographs its programs and participants.  In order to photograph minors, we must have parental/guardian permission.  

Please read the following release and sign it for the child participant(s) in this program:  Visions of Windsor Summer Camp, August 11 – 15, 2008. 

I hereby agree and consent to the use of any pictures taken during Visions of Windsor Summer Camp at the Windsor Historical Society.  The Society shall have the right to use, reproduce, copyright, distribute, and provide access to the photographs for any purpose whatsoever and may use each of the undersigned’s name and likeness in connection therewith.  I understand that these images may be used at any time without re-contacting me.  I agree to waive all compensation for such use.
